The England and Saracens second-row was forced to have five stitches to an eye wound suffered after Azam kicked out at him during a Guinness Premiership match at Vicarage Road on September 27th.
Despite succeeding in his appeal to have the punishment downgraded from a top level offence to a mid-range level, the 12-week period was upheld due to Azam's previous poor disciplinary record – he has only just returned from a nine-week ban for eye gouging an opponent last season.
The original judgement of a flat three-month period out was changed to an eight-week ban with six weeks added on for his previous indiscretions. That was then reduced by two weeks for his guilty plea and other mitigating factors.
It means Azam is unavailable until December 29th.
