A report released on Monday said the services were 'not fit for purpose' and urgent action was required to improve the care.
The investigation was launched after inspectors described weaknesses in the care offered to children at risk of serious physical or sexual abuse.
It is believed 15 children known to social workers have died in the city in the past five years.
Today's report claims there has been a 'decade of underperformance' with very little change to the services provided.
A lack of senior management, staff shortages and 'shocking' accommodation were all highlighted.
The report, led by former city councillor Len Clark, was commissioned after an urgent review of children's services was launched around the country following the death of Baby Peter in Haringey.
Among the deaths investigated in Birmingham was that of seven-year-old Khyra Ishaq, from Handsworth, who allegedly starved to death.
Her mother and stepfather are due to stand trial for her murder next year. They deny the charges.
