The blast took place close to a Shia Muslim shrine popular with Iranian pilgrims.
Iranian media said the bus exploded while refuelling just 500m from the shrine of the granddaughter of the Prophet Muhammad.
It was reportedly empty of passengers, with the Iranian bus driver and his assistant as well as four bystanders dying in the explosion.
Dozens of others were injured in the blast, reports said.
Terrorist attacks in Syria are rare, but Thursday's explosion, blamed on a faulty gas canister in the luggage of Iranian pilgrims, coincides with the visit of senior Iranian security official Saeed Jalili, who was in Damascus for talks with his Syrian counterparts.
